Student Request Form

This form is for teachers who have found a student in the Open Free Trials spreadsheet or through a student request email and would like to request a specific student.

NEW: You can now specify which instruments you can teach in the event a family requests multiple instruments but you can only teach a few. Feel free to request any family that includes your instrument(s).

Please be sure to include the correct student code!

Tip: If a student’s availability does not line up with yours, send a request anyway with YOUR perfect slot. You never know, it might work out!
