Trial Lesson Report
This form is for reporting the trial lessons you’ve completed. Though they’re free for the family, you are paid $20 per half hour trial. Please make sure to submit it for every trial you do (whether or not the family enrolls) so our Payment Manager is aware of exactly how many to pay you for!
